Plan Home Care is a non-skilled in-home care provider of upstate SC.

A simple, affordable and efficient solution to finding local transportation for people of all ages.  Book rides at the call center toll free, using a desktop or laptop computer, or via the Feonix App on a smartphone.  A one-stop-shop for:

1) Finding available transportation options   2) Scheduling and booking using a preferred mode of transportation   3) paying in advance

The Columbia Rehabilitation Clinic, Inc. is a comprehensive outpatient rehabilitative facility dedicated to the treatment of patients in need of physical restoration. Established in 1977, our clinic was the first private outpatient physical therapy and rehabilitation facility in Columbia, South Carolina.

Our rehabilitation clinics provide physical rehabilitation and therapy services for disorders that are orthopedic, neurological, arthritic, pulmonary, cardiovascular, and sports related. All our rehabilitation services are provided on a one on one, individual basis, not group sessions, and we treat adults as well as adolescents (age 6 and up for orthopedic problems).

The primary goal of our clinics is to provide the patient, through physical therapy, an opportunity to achieve his or her maximum level of independence. Patients can be treated at the clinic on a referral from a physician, dentist, or podiatrist. A patient can also be seen for 30 days without a physician’s referral.

Club Horizons is a day program specifically designed for adults with disabilities.
We strive to provide a loving, compassionate, safe and educational environment with our members. Our “Club” theme is designed to entertain and inspire all who share their time with us. The program focuses on “person centered planning." Our goal is to encourage each participant to reach their personal potential, while at the same time enjoying the benefits of social interaction in a happy and stimulating environment.

Inpatient Geriatric Behavioral Health

We are an inpatient crisis stabilization unit for patients 65 and older who are experiencing a mental health crisis or dementia related behavioral crisis and who are acute enough to warrant hospitalization.  We are a secured unit with 20 beds located inside KershawHealth Medical Center in Camden, SC.  Our Medical Director is Geriatric Psychiatrist, Kevin Nasky, DO and our staff consists of Dr. Nasky, a psychiatric nurse practitioner, social workers, activities therapist, RNs, LPNs and CNAs and a community education coordinator.  Our average length of stay is 10-12 days.  We accept all insurances at this time.

Protection and Advocacy for People with Disabilities Inc. (P&A) is a statewide non-profit working to protect and advance the rights of people with disabilities. P&A provides information and referrals, training, abuse and neglect investigation, legal representation, and advocacy for policy change.

Our mission is to create resourceful and resilient family caregivers through innovative programs that offer education, support and wellness. Our programs and services are provided free of charge.

The Colleton county on Aging, Inc.'s mission is to address the cognitive, social/emotional and physical needs of the senior citizens of the county. This Nonprofit Human Services Agency provides hope, encouragement and support through an array of services designed to meet the individual client's needs. Emphases are placed on providing home delivered meals and service that promote health, wellness and preventive care.

Family Justice Center of Georgetown and Horry Counties is a unique collaboration of community partners and agencies working together to provide a safe and welcoming environment for the victims of domestic violence, sexual assault and child abuse.

Provides SNAP and TANF benefits for those eligible. Investigates Child and Vulnerable Adult abuse and neglect.
