Dementia Dialogues® is a nationally registered and evidence-informed, intervention program. This 5-module training course is designed to educate community members and caregivers (formal or informal) for persons who exhibit signs and symptoms of Alzheimer’s disease and related dementias (ADRD). The modules cover valuable information and recommendations including an overview of dementia, strategies for effective communication, understanding the environment and ways to promote independence, addressing challenging behaviors, and creative problem solving. Dementia Dialogues® is offered nationwide by Certified Instructors in-person and virtually, and as a virtual self-guided training at no cost to participants. Dementia Dialogues® is provided through Office of the Study of Aging at the Arnold School of Public Health, University of South Carolina.

We provide Guardianship, Guardian Ad Litem, Conservator, and Fiduciary services for vulnerable or incapacitated individuals.  We have been providing these services since 2015 in South Carolina (and can also provide services in North Carolina).  

Care Partners LLC is owned by Seth Zamek.  Seth holds a Masters in Healthcare Administration, is a Fellow in the American College of Healthcare Executives, is a licensed Emergency Medical Technician and a Certified Dementia Care Specialist.  Seth has over 30-years experience in the healthcare and guardianship field, most recently as the owner and Executive Director of Senior Helpers.  He has worked extensively in the Probate Courts and with Probate Court Judges and staff in South Carolina.  Seth currently serves on the Boards of Directors for the SC Alzheimer's Association, the Parkinson Association of the Carolina's, the SC Home Care and Hospice Association, and the SC Commerce Department's Catawba Regional Education Center.

Zamek is personally involved in every client case Care Partners LLC agrees to take on.  The organization serves vulnerable or incapacitated individuals of all ages and prides itself on being very responsive to the Courts, Attorneys, families petitioning the Court and clients.

Faithful Touch Home Care, LLC strives to offer excellent and affordable community-based home health care to individuals and families of South Carolina. Our goal is to employ competent, caring, and well-trained individuals who are responsive to the needs of our patients, their families, and the communities we serve. Each staff member will meet the State of South Carolina's educational and training requirements for their services, and we encourage and support the continued education of each service provider. Comprehensive resource and referral directory for Seniors and Caregivers. Covering the Upstate, Greater Charleston, Midlands and Charlotte markets (including SC counties surrounding Charlotte).

Resources can be found in our printed magazines and through our online directory.  Both items also include articles and other important information for seniors and caregivers.

Specialize in senior placement referrals into Independent Living, Assisted Living, and Memory Care Communities for Senior Adults. We meet in person with the potential client/family to gather information about their care needs, budget, location, and personal preferences about a community. We determine what communities will meet their needs, then arrange and accompany them on tours. 

We have assessed, and continue to monitor, over 25 private-pay independent, assisted living, and memory care communities throughout Greater Columbia that offer a wide range of budgets, care levels, and amenities. We also can assist your Veteran patients and their spouses with the initial evaluation for possible Aid & Attendance benefits. Oftentimes, if they are eligible, it can make private-pay communities an affordable option. There is no charge to the patient or their family for this personalized service. 

Assisted Living Locators is a FREE Compassionate referral and placement service for families and seniors looking for Independent Living, Assisted Living, Memory Care or In Home Companion Care Services. Assisted Living Locators Care Advisors are in your community and work closely with you to guide you through the process from beginning to end—answering your questions and addressing your concerns, helping you feel confident and comfortable with your decisions. We are experts with the care options in your area. The COMET, officially the Central Midlands Regional Transit Authority (CMRTA), is a regional transportation authority formed by Richland County, City of Columbia, and Lexington County by the Central Midlands Council of Governments.
 

The COMET provides transit services on 47 fixed routes, five ReFlex services, vanpool service for commuters, subsidy programs with Uber and Lyft and DART for transportation to grocery stores and at night, ADA complementary paratransit service throughout Richland and portions of Lexington Counties and eight Blue Bike bikeshare stations in Downtown Columbia. The COMET transports approximately 2.2 million passenger trips a year on a fleet of more than 90 buses, vans and trolleys. Transit services are provided under contract with RATP Dev USA, Inc., Enterprise Holdings, Lyft, Uber and Bewegen.
